Refer a Child to A to Z Child Development
If you have concerns about the development of a child under age 3, our team can help you begin the Early Intervention referral process.
Eligibility is determined through the Early Intervention evaluation process. Services are identified through the Individualized Family Service Plan (IFSP) process and authorized by the municipality. We can help your family begin the referral process. Submitting our form or calling the agency does not by itself establish eligibility or authorize services.
